Minimum age - 21 years (if under 21, parental consent is required in the form of an Affidavit stamped by a notary) |

Passport copies (pages containing passport number, photo, issue/expiry dates) |

Birth certificates |
|
If divorced - Decree Absolute |
|
If widowed - former spouse's Death Certificate and previous Marriage Certificate |
|
If adopted - Adoption Certificate |
|
If name changed by Deed Poll - legal proof required, stamped and signed by a solicitor |

Photocopies of all documents must be stamped by a notary or solicitor. The notary must verify that they are true and precise copies of the original, and sign and date every copy. The notary's or solicitor's signature and stamp must be in ink |
